In case of an illness or injury, I authorize DogsSocial LLC to seek appropriate

medical treatment for my pet. I understand that every effort will be made to take my pet to the Veterinarian Clinic specified above if the situation permits; however, DogsSocial LLC has the authority to seek treatment at any veterinary clinic. I accept full responsibility for all fees and charges incurred in treating my pets.

1. Prices are subject to change. Payment for all services is required in advance. Cash and cash app payments are accepted, as are credit card payments through Stripe.

2. DogsSocial LLC reserves the right to not accept a dog into daycare, boarding, or any pet service for any reason. All DogsSocial LLC rules are subject to change at DogsSocial LLC's sole discretion.

3. I, the undersigned, expressly agree to be held responsible for any damage or cost

incurred by my dog(s), including but not limited to any injury inflicted upon another dog because of a fight, rough play, bullying, damage to property, or injury to a staff member. If veterinary or emergency care is required for injuries caused by my dog, I understand I will be notified of the incident, the steps taken, and the cost incurred.

4. DogsSocial LLC agrees to exercise due and reasonable care for my dog(s). I

release DogsSocial LLC from any claims made against DogsSocial LLC or

losses or damages of any kind suffered by DogsSocial LLC for or attributable to,

any illness or injury of dog(s). I agree that I shall be responsible for the costs of injury to DogsSocial LLC, Paul Soliz, other staff, other dog(s) or damage to facility

caused by my dog(s). Also, if DogsSocial LLC transports my dog(s), I will not hold

DogsSocial LLC responsible for any injury or accident during transportation.

5. I recognize an inherent risk of injury or illness in any environment associated with

cage- less doggy daycare. In group play environments, like that of DogsSocial LLC. I

understand that dog(s) sometimes sustain minor injuries such as nicks, scratches, and sprains due to play. I agree that DogsSocial LLC will treat such minor injuries and not notify me upon my return. I also understand that if a more serious injury occurs and veterinary attention is necessary, I will not hold DogsSocial LLC liable for such an injury. I will also be responsible for any cost associated with said injury. If your dog becomes ill or injured while in the care of DogsSocial LLC, we will attempt to contact you using the information you provided to DogsSocial LLC.

6. I certify that my dog(s) is/are in good health and has/have not been ill with any

communicable diseases in the 30 days before check-in. My dog(s) are receiving a form of flea/tick and heartworm control and does not suffer from any illness, disability or condition which could affect the dog or the safety of other dogs at DogsSocial LLC. I have also provided DogsSocial LLC with a copy of my dog(s) most current vaccination records. I will continue to do so as vaccination records change and get updated.

7. I understand and agree that in bringing my dog(s) to DogsSocial LLC, the

representation and information given about my dog(s) health and behavior have been truthful and complete. I agree that I have fully disclosed all information relating to any aggressive behavior of my dog(s). If aggressive and/or bullying behavior begins or continues, DogsSocial LLC reserves the right to remove the dog(s) from group play.

8. I certify that I have made full disclosure and have carefully read, understand, and

accept the terms and conditions stated of this DogsSocial LLC Daycare and

Boarding agreement and acknowledge that this agreement shall be effective and

binding upon parties.

Dog Walking, Vacation Care, and other services

1. DogsSocial LLC will endeavor to offer sound, safe and responsible care for my

dog(s). However, I recognize that DogsSocial LLC and its employees are not

responsible for unintentional errors, omissions or incorrect assertions. I am and will

always remain responsible for the actions of my dog(s) and hereby agree to indemnify and hold harmless DogsSocial LLC of any claims of injury, expense, costs, or damage caused by the action of my dog(s) while under DogsSocial LLC’s care. I understand and acknowledge the inherent risks of owning a dog(s), including but not limited to the risk of dog bites to myself or others. Additionally, DogsSocial LLC will act respectfully and cautiously in my home in my absence. I hereby agree to indemnify and hold harmless DogsSocial LLC and its employees of any and all damage claims to my house.

2. DogsSocial LLC and its employees will do their best to keep my dog(s) safe while

walking and waste removal services. However, I also understand the risk of encountering other dogs on walks in public. I will not hold DogsSocial LLC and its employees responsible for injury or damage by another dog beyond their control.

3. Prices for Services are detailed in our Services and Pricing Guide and are subject to change. Payment is collected at the beginning of the week or on the first day of service for each week and must be made regularly and in a timely manner. Cash, credit cards and cash app payments are accepted.

4. DogsSocial LLC cancellation policy; Cancellation policy for Dog Walking; DogsSocial LLC requires 24 hours; notice in the event of a last-minute cancellation; the Client will be responsible for the payment of the visit unless DogsSocial LLC is notified in a timely manner.

5. In the event of a personal emergency or illness of the primary Dog Walker/Pet Sitter; I authorize DogsSocial LLC to arrange for another qualified person to fulfill the

responsibilities outlined in this agreement

6. I authorize emergency medical care to be provided by the veterinarian named on the vet release or an appropriate alternative to be determined by DogsSocial LLC if my regular veterinarian is not available or that closer care is required. I will reimburse DogsSocial LLC for any charges related to emergency care, including but not limited to transportation, prescriptions or an additional fee.

owner, release DogsSocial LLC from any claims made against DogsSocial LLC or

losses or damages of any kind suffered by DogsSocial LLC for or attributable to, any

illness or injury of dog(s). I agree that I shall be responsible for the injury costs to

DogsSocial LLC, Paul Soliz, other staff or other dog(s). Also, if DogsSocial LLC

transports my dog(s), I will not hold DogsSocial LLC responsible for any injury or

accident during transportation.
